This will be done in conjunction with your GP and any other healthcare professionals providing support
  • If you then decide to go ahead with care, our team will match your preferences to the right carer

Step 3: Fully managed, flexible care

  • Once care has started, we will contact you regularly and your Care Manager will visit your loved one. We routinely adapt care plans to ensure they are flexible and match changing needs
